 Bright Futures Program

The Bright Futures program is a complete perinatal outreach program that focuses on pregnant women who are receiving no prenatal care. Goals of the program include helping women to access prenatal care, have a healthy pregnancy and after the baby's birth, teaching infant care and building parenting skills.  Safe Beginnings Program

This program serves MRDD (mentally retarded / developmentally disabled), mentally ill and physically challenged pregnant or new mothers. Goals include reducing premature birth preventing child abuse and neglect and promoting good parenting skills. The intensity of the program is dependant upon the individual needs of the family. The Safe Beginnings program continues until the participant enters a long term care program. Read More

 Bridge to the Future Program

The Bridge program serves as a "Bridge" between hospital and long term community services for medically fragile infants. Services are based on individual family strengths, and are designed by the family and the nurse. This program was featured in the 1996 "Models That Work" Compendium of Innovative Primary Health Care Programs for Underserved and Vulnerable Populations. Read More

 Teen Parent Program

This program serves first time mothers under age 19. It is an eighteen month program. The goals of the Teen Parent program include reducing repeat pregnancies, infants receiving all immunizations, and reducing accidents by educating young mothers on what safety hazards are to the infant at each stage of development. The program also focuses on increasing young mother's awareness of the signs of physical illness in the infant and making her aware of the health resources in her community. Read More
